Deceased donor liver transplantation from donors with central nervous system malignancy: Experience of the Inonu University

INTRODUCTION: Liver transplantation from deceased donors with a central nervous system (CNS) malignancy has some risk of tumor transmission to the recipient. Though the risk is small, this group of donors is regarded as marginal. The use of marginal grafts may be an acceptable alternative practice in order to expand the donor pool in countries where there is a shortage of donated organs. The aim of this study was to examine and present the outcomes of liver transplantations performed using donors with a CNS tumor.
METHODS: Between March 2002 and July 2017, 1990 (deceased donor: n=399, 20%; living donor: n=1591, 80%) liver transplantations were performed at the center. Of the 399 deceased donors, 17 (4.2%) had a CNS tumor. The data of donors with a CNS tumor and of recipients who survived for more than 1 month (n=11) were retrospectively reviewed. Demographic data, the grade of the CNS tumor, tumor transmission to recipient data, and survival rates were analyzed.
RESULTS: Only 2 (18%) grafts were provided locally, 6 (54%) were offered to the transplantation center after all of the national centers had declined them, and 3 (37%) were made available to us by the national coordination center for patients with a documented notification of urgency. High-grade (grade III-IV) brain tumors were detected in 7 (64%) donors, while low-grade (grade I-II) tumors were found in 2 patients. The remaining 2 donors were not pathologically graded because the diagnosis was made radiologically. The 1-, 3-, and 5-year overall and tumor-free survival of the patients was estimated at 100%, 70%, and 45%, respectively.
DISCUSSION AND CONCLUSION: A median survival of 40 months (range: 13-62 months) was achieved in recipients of grafts from a donor with a CNS tumor and no donor-related malignant transformation was observed.

Merkezi sinir sistemi tümörlü kadaverik donörlerden karaciğer nakli: İnönü deneyimleri

GİRİŞ ve AMAÇ: Merkezi sinir sistemi (MSS) tümörlü kadavralardan çıkartılarak gerçekleştirilen karaciğer naklinde, düşük de olsa, alıcıda donör kaynaklı malignite gelişme riski olduğu için, bu tür donörler marjinal kabul edilmektedir. Marjinal greft kullanımı organ bağışının az olduğu ülkelerde donör havuzunu genişletmek için tercih edilen bir uygulamadır. MSS tümörlü donörlerden gerçekleştirdiğimiz karaciğer nakli sonuçlarını sunmayı amaçladık.
YÖNTEM ve GEREÇLER: Mart 2002 – Temmuz 2017 tarihleri arasında merkezimizde 1990 (n=399, % 20 kadavradan, n=1591, % 80 canlı vericiden) karaciğer nakli gerçekleştirildi. Bu 399 kadaverik karaciğer naklinin 17’si (% 4,2) MSS tümörlü donörlerden gerçekleştirildi. MSS tümörlü donörlerin ve bu greftlerin takıldığı ve bir aydan uzun süre yaşayan 11 alıcının verileri geriye dönük olarak incelendi. Demografik verileri, MSS tümör evreleri, alıcılarda tümör geçişi, sağ kalım analizleri yapıldı.
BULGULAR: Tablo 1 ve 2 de sırasıyla alıcı ve MSS tümörlü donörlerin demografik özellikleri sunulmaktadır. Bu greftlerin sadece 2’si (% 18) kendi bölgemizden sunulmaktayken, 6’sı (% 54) ulusal hiçbir merkez tarafından kullanılmadığı için son sıradan merkezimize sunulmuş, 3’ü (% 27) ise merkezimizdeki acil bildirimi olan hastalar için ulusal koordinasyon merkezi tarafından merkezimize sunulmuş ve transplantasyon gerçekleştirilmiştir. Yüksek derece (grade III – IV) beyin tümörü 7 (% 64), düşük derece (grade I – II) ise 2 donörde saptanırken, 2 donörün tanısı radyolojik olarak konulduğu için patolojik olarak evrelendirilemedi. Genel ve tümörsüz sağkalım 1-3-5 yıllık sırasıyla, %100-70-45 saptandı.
TARTIŞMA ve SONUÇ: MSS tümörlü greftler kullanarak gerçekleştirdiğimiz karaciğer nakli alıcılarında medyan 40 ay (13-62) sağkalım elde edilmiş ve hiçbir hastada donör kaynaklı malign transformasyon izlenmemiştir.
