Effect of sugammadex, rocuronium and sevoflurane on oxidative stress and apoptosis in cerebral ischemia reperfusion model in rats

OBJECTIVE: Cerebral ischemia-reperfusion (I/R) injury causes neurological dysfunction and cell death. Sugammadex, as a large molecule, is normally difficult to pass through the blood-brain barrier (BBB). In ischemia, molecules can pass into the brain tissue. In this study, we aimed to evaluate the effect of sugammadex in the presence of cerebral I/R damage in rats with a general anesthesia model with sevoflurane and rocuronium.
METHODS: Rats were divided into 7 groups; Group 1 (Control), Group 2 (Sham), Group 3 (Sevoflurane), Group 4 (Sugammadex), Group 5 (Sevoflurane + Rocuronium), Group 6 (Sevoflurane + Sugammadex), Group 7 (Sevoflurane + Rocuronium + Sugammadex). Brain tissues of rats with cerebral I/R damage with bilateral carotid occlusion were removed. Tissue Malondialdehyde (MDA), Myeloperoxidase (MPO), and Superoxide dismutase (SOD) levels were examined with ELISA and apoptosis was examined by Caspase-3.
RESULTS: The number of caspase-3 positive cells decreased the most in Group 4 compared to the other groups. Group 4’s mean MDA and MPO levels were lower than Group 2. There was no significant difference in terms of SOD levels.
CONCLUSION: The apoptotic effect of sugammadex was lowest compared to other agent groups, and it did not increase oxidative damage as much as the other groups.

Sugammadeks, rokuronyum ve sevofluranın serebral iskemi reperfüzyon modeli oluşturulan ratlarda oksidatif stres ve apopitozis üzerine olan etkisi

Amaç: Serebral iskemi reperfüzyon hasarı, nörolojik fonksiyon bozukluğuna ve hücre ölümüne kadar giden ciddi bir sürece neden olmaktadır. Sugammadeksin büyük bir molekül olarak normalde kan beyin bariyerinden geçişi zordur. İskemide ise KBB bozulur ve normalde geçişi mümkün olmayan moleküller beyin dokusuna geçebilir. Bu çalışmada sevofluran ve rokuronyum ile genel anestezi modeli oluşturulan ratlarda serebral I/R hasarı varlığında sugammadeksin etkisini değerlendirmeyi amaçladık.
Metod: Ratlar 7 gruba ayrıldı; Grup 1 (Kontrol), Grup 2 (Sham), Grup 3 (Sevofluran), Grup 4 (Sugammadex), Grup 5 (Sevofluran + rokuronyum), Grup 6 (Sevofluran + sugammadex), Grup 7 (Sevofluran + rokuronyum + sugammadex). Bilateral carotis oklüzyonu ile serebral I/R hasarı oluşturulan ratların beyin dokuları çıkarılıd, Kaspaz-3 ile apopitozis ve doku MDA, Myeloperoxidase (MPO ve Superoxide dismutase (SOD) düzeyleri incelendi.
Bulgular: Kaspaz-3 pozitif hücrelerin sayısı diğer gruplara göre en fazla Grup 4’te azalma gösterdi. Kaspaz-3 pozitif hücrelerin sayı artışı ortalamalarına göre düzenlenişi sırasıyla, Grup 1, Grup 4, Grup 7, Grup 6, Grup 5, Grup 3 ve Grup 2 olarak belirlendi. Grup 4’ün ortalama MDA ve MPO düzeyleri Grup 2’ye göre daha düşüktü. SOD düzeyleri açısından ise anlamlı bir farklılık bulunmadı.
Sonuç: Sugammadex uygulamasının I/R hasarına sekonder olarak meydana gelen apopitozis üzerinde olumlu etki yaptığı, diğer ajanlara göre apopitotik etkisinin en düşük seviyede olduğu ve oksidatif hasarı diğer gruplar kadar artırmadığı tespit edildi. (NCI-2022-9-6)
